1. DEATH CERTIFICATE
- (Todesbescheinigung) issues a doctor after confirmation of death.
2. GERMAN DEATH CERTIFICATE
(Sterbeurkunde) is issued by the Registry Office in Germany.
3. PERMISSION OF THE CONSUL OF THE REPUBLIC OF POLAND TO TRANSPORT A BODY OR URN TO POLAND
- Issued by the consul at the embassy.
4. AUTHORISATION BY THE GERMAN AUTHORITIES TO EXPORT THE BODY OR URN TO POLAND
- (Leichenpass)
